Normalize Flat Appointments into ScheduleApp Appointments

This process reads appointments from ScheduleApp_Plain.APPOINTMENTS, resolves each record against normalized PATIENTS and the Event / Opportunity Interface, converts DaySmart dates and times, and synchronizes the final records into ScheduleApp.APPOINTMENTS.

Run Patients normalization and the Event / Opportunity Interface first. Appointments with a missing Patient or unresolved Opportunity are still preserved and can be completed automatically in a later normalization run.
